We offer online, telephone and face to face consultations. In special circumstances you may be eligible for a home visit.
To book an appointment:
- Phone us Monday to Friday, 8:00am to 6:30pm on 01507603121 and speak to our Care Navigation team or use our automated service.
- Use our online form Monday to Friday and we will respond within 24 hours. We will pause this service when there is high demand.
- Book an appointment via SystmOnline or via the NHS App.
- Visit the surgery and speak to one of our Care Navigators from Monday to Friday, 8:00am to 6:30pm.

Urgent appointments
If you need to see a clinician on the day please contact the surgery using one of the above methods.
If you have a life threatening emergency such as chest pains,
shortness of breath or severe injury please contact 999.
Routine appointments
For general health queries please contact the surgery for a routine appointment.
When the surgery is closed
If the situation is an emergency or life threatening, dial 999 and ask for an ambulance.
If you need urgent medical advice when the surgery closes go to 111.nhs.uk. The surgery is closed between 6:30pm and 8:00am weekdays, during weekends and Bank Holidays.
Call 111 if you cannot use the online service or because you:
- need help for a child under 5
- have complex problems caused by an existing medical condition
- need to get end-of-life care
Cancelling an appointment
- Use your NHS App or SystmOnline account 24/7.
- Call the surgery on 01507 603121 to use our automated system Patient Partner 24/7.
- Phone us Monday to Friday, 8:00am to 6:30pm on 01507603121 and speak to our Care Navigation Team.
- Visit the surgery and speak to one of our Care Navigators from Monday to Friday, 8:00am to 6:30pm.
Changing an appointment

Home visits
Home visits are only available to patients who are:
- end of life
- bed-bound and travel to the surgery would cause a decline in their condition
Our guidelines are in accordance with the National and local recommendations.
If you are eligible for a home visit please call the surgery between 8:00am and 10:30am on 01507 603121 and speak to a member of our Care Navigation Team.
Evening and weekend appointments
Evening and Weekend appointments are available at this GP practice or at an NHS service nearby. To book at these times, contact the Surgery. This practice is a member of the Meridian Medical Primary Care Network (MMPCN) and supports the delivery of Enhanced Access Provision. We work collaboratively to deliver these services with other member practices from Tasburgh Lodge Surgery, James Street Surgery and Marsh Medical Practice.
Lincolnshire and District Medical Services (LADMS) is a private limited company working in partnership with Meridian Medical PCN to provide NHS out of hours GP appointments through the Enhanced Access Service; also known as Enhanced Access Hubs. Although, LADMS is separate to your GP practice, its purpose remains to deliver the same high standard of patient centred health care that you receive via your local GP.
Following your appointment with the Enhanced Access Service, you may be referred to a healthcare provider for further treatment. Should this be necessary, LADMS will send you a covering letter and referral information explaining next steps via Royal Mail; LADMS contact details will be provided on the letter should you have any queries.
The opening hours are 18:30 to 20:00 Monday to Friday and 09:00 to 17:00 Saturdays (closed for bank holidays). The service is based at Woldside, County Hospital, Louth.
